PROGRAM AND POSITION SUMMARY:<\/strong><\/p>\n REPORTING TO: \u00a0Senior Manager, Community Support<\/p>\n NATURE AND SCOPE: The Complex Care Coordinator Assistant is available in our Senior Services department, under the direction of the Senior Manager, Community Support. This position will provide a range of challenging experiences, including working with frail, vulnerable and isolated elderly clients. The youth will work directly with our Complex Care Coordinator to help support our clients to live in their own homes as long as possible.<\/p>\n DUTIES<\/strong>:<\/p>\n \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Work with senior clients living in the community of Beaches-East York and assisting in their case management<\/p>\n \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Escort clients to medical and non-medical appointments<\/p>\n \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Friendly visiting\/ home visits and client shopping as required<\/p>\n \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Assisting with client assessment and reassessments using the InterRAI CHA tool<\/p>\n \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Liaising with families, other Agency staff, external stakeholders, community agencies and local businesses<\/p>\n \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Entering and documenting data accurately into a database (CIMS experience preferred)<\/p>\n \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Attending team meetings<\/p>\n \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Providing administrative support including Intake duties as required<\/p>\n \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Perform other duties, appropriate to the position, as required.<\/p>\n In order to apply for this position, you\u00a0must<\/strong>\u00a0be 15 to 30 years of age.
